Helicopter Rental Agreement This agreement made and entered this date / / between Centennial Helicopters, Inc. at 1 Wallingford Road #3, Danbury, CT 06810 and (Name of renter) (Address) (Phone) (City) (State) (Zip) WHEREAS, Centennial Helicopters, Inc. is the owner, operator, or manager of certain training or rental aircraft: AND WHEREAS, Renter seeks to Rent aircraft from Centennial Helicopters, Inc. under terms and provisions of this agreement. THEREFORE, the parties hereto agree as follows: 1. OPERATION: Renter covenants and agrees with Centennial Helicopters, Inc. to abide by the following rules and regulations in the operation of the aircraft: a. To follow all FAA regulations applicable at the time of flight; b. To operate within the limits of the Aircraft Pilot Operator Handbook; c. For the Robinson R22

helicopter, no pilot shall carry passengers until the pilot has logged at least seventy five hours in helicopters, of which at least twenty hours in the R22, of which at least five PIC hours are in the Robinson R22 AFTER receiving their helicopter rating. d. For the Robinson R44 helicopter, no pilot shall carry passengers until the pilot has logged at logged at least one hundred hours in helicopters, fifty hours in Robinson helicopters, of which at least five PIC hours are in the Robinson R44 after receiving their helicopter rating OR two hundred hours in helicopters of which at least ten PIC hours are in the R44 after receiving their helicopter rating. e. For the Schweizer 300C helicopter, no pilot shall carry passengers until they have logged three PIC hours in the Schweizer 300C helicopter AFTER receiving their helicopter rating. f. To use the aircraft for personal pleasure and not to engage in commercial operation of any kind; g. To take off and land at open public use FAA approved

Airports; h. Not to engage in formation flight; i. Not to give flight instruction, or receive from other than a company pilot; j. To undergo a flight proficiency check for each make and model of aircraft available for rent, and every 90 days thereafter of the last flight for the appropriate make and model. k. Not to permit smoking the aircraft; l. To leave the aircraft clean and free of garbage; m. The following operations are prohibited unless a company CFI is on board or an emergency exists or the Pilot in Command has a written endorsement from Centennial Helicopters, Inc. 1. 2. 3. 4 5. 6. 7. JUL15-17 MAINTENANCE: 1 Renter covenants and agrees with Centennial Helicopters, Inc. to use the aforesaid aircraft in the following manner: a. To report maintenance discrepancies to Centennial

Helicopters, Inc promptly; b. To obtain approval in advance from Centennial Helicopters, Inc for any emergency repairs to be performed during the term of this agreement; c. To have repairs made by an FAA certified mechanic; (Renters signature) 3. SCHEDULING: Renter agrees that the aircraft will be scheduled for use pursuant to the following: a. Scheduling shall be only for the time reasonable necessary for the purposed flight If the renter is more than 15 minutes late to pick up the aircraft after a reservation of same has been accepted, then the Centennial Helicopters, Inc. shall have the option of re-renting the aircraft to third party without any claim of right to use the aircraft by renter. Additional charges may incur under such circumstances pursuant to the provisions hereinafter contained; (see 4.) b. To leave an itinerary or flight plan with the Centennial Helicopters, Inc prior to departure and to notify the Centennial Helicopters, Inc. in the

event of any change and from said scheduled itinerary (Renters signature) 4. PAYMENT: The renter covenants and agrees to make payment in accordance with the following: a. To pay the current posted wet rate rental fee on Hobbs time and instructor fees based on Hobbs time plus ground instruction time; b. To pay all charges incurred at the time of service by cash, check, or major credit card (only if acceptable to Centennial Helicopters, Inc.) c. To leave a credit card on file or have money on account in excess of $25000 to satisfy charges that can not be paid due to a late return. (Please see attachment) d. To pay all landing fees and charges at other airports; e. To return aircraft to Danbury Airport and be responsible for any expenses incurred in connection with returning aircraft to Danbury; f. Cancellations require forty-eight (48) hours notice by Renter or Renter will be charged 30 percent of aircraft time booked and 50 percent of instructor time booked.

To pay a minimum fee for rental of: 30 percent of time booked on weekdays and 50 percent of time booked on weekends (based on hours Centennial Helicopters, Inc. is open) h. To pay “No Show” charges for aircraft scheduled but not used, except of unforeseen weather or maintenance. The “No Show” charge is 50 percent of scheduled aircraft time and 100 percent of scheduled instructor time: i. A “Late Charge” in addition to the minimum set forth above based on the current fees set forth in Paragraphs a. and e above for each such hour (or portion thereof) in which the plane shall be held beyond the scheduled time of return. (Renters signature) 5. STUDENT PILOTS: If the Renter is a student pilot, said Renter covenants and agrees as follows: a. To conduct solo flights, he must be approved by a Centennial Helicopters, Inc Service flight instructor; b. To conduct pattern work, weather minimums of

five (5) miles visibility and 3,000 feet ceiling must be in effect; JUL15-17 2 c. To fly beyond the pattern, flight minimums of seven (7) miles of visibility and 5,000 feet of ceilings must be in effect; d. To fly cross country, a flight plan must be filed with Centennial Helicopters, Inc; and Flight Service, with visibility of eight (8) miles and ceilings of 5,000 feet minimum; (Renters signature) 6. FUEL: Any fuel purchased in connection with the use of the aircraft pursuant to this agreement from an off-site source will be credited against the charges set forth in Section 4(a) and 4(e) herein before set forth provided that accurate receipt for fuel purchases are presented upon return of the aircraft. INSURANCE: The Centennial Helicopters, Inc. covenants and agrees that it is insured for liability and hull damage with a $8,750.00 deductible for the Schweizer 300C, $11,25000 for the Robinson R22

Beta II and $32,50000 for the Robinson R44. The renter shall be responsible for payment of said deductible or any part thereof for damages incurred while operating the aircraft. In the event that the aircraft is operated in violation of this agreement or the rules and regulations of Centennial Helicopters, Inc. pertaining to other aircraft, persons, or property. The Renter agrees to indemnify and hold harmless the Centennial Helicopters, Inc against loss or liability for damage or injury to person or property in excess of the amount of insurance coverage in force pursuant to this agreement. Centennial Helicopters, Inc will, upon request from Renter, provide Renter with a statement of liability coverage pertaining to the aircraft to be leased in accordance with the terms of this agreement. Renter hereon waives any claim for loss, liability, or damage against Centennial Helicopters, Inc. arising out of the use of the aircraft pursuant to this agreement Centennial Helicopters, Inc.

strongly recommends the purchase of “Renters Insurance” to all of our customers. (Renters signature) 8. ENTIRE AGREEMENT: This agreement sets forth the entire understanding between the parties hereto and shall not be modified except as in writing between Centennial Helicopters, Inc. and Renter The Renter covenants that he has read and understands the terms and provisions of this agreement and will comply with same in renting aircraft from Centennial Helicopters, Inc. (Renters signature) 9. TERM: The term of this Lease shall be for NINETY (90) days from the date hereof but shall automatically renew for a similar period provided that the Renter leases a plane from the Centennial Helicopters, Inc. within said ninety (90) day period and that the Renter remains in good standing within the terms of this agreement and flight school policies. Either party may terminate this Lease upon written notice to the other This agreement shall

be amended from time to time as it shall apply to different aircraft to be leased from Centennial Helicopters, Inc.
