NAVY GENERIC LETTER OF REQUEST WEAPONS SYSTEM CHECKLIST 1. The LOR must be submitted in writing in English (translated if not) 2. Which Purchasers Military Service is submitting request?? (Army, Navy, Air Force) 3. Estimated procurement date?? 4. Does request include Sole Source?? 5. What source of funding will be used?? (FMF, National Funds, etc) a. Type of Assistance Code: b. Delivery Term Code: c. Mark For Code: 6. Is this an initial fielding to Country?? a. If not, list previous FMS cases and/or DCS transactions; Identify FMS case or export license number if known. 7. What additional capabilities may be added to this platform / equipment?? 8. What weapon system is being requested?? (ie: FA-18 Super Hornet block 28, Sidewinder AIM-9X IR missile, General Purpose MK-82, 500lb Bombs, P-3C Maritime Patrol aircraft, ARC-410 Radios, MH-60 Helicopter, AN/ALQ-144A(V) 1 Countermeasures Set, etc) a. Provide as follows: (1) Equipment nomenclature or description. (2) Part # and national stock #

if known. (3) Quantities. (4) Intended end use (type aircraft, ship, weapon to be used on or with, if known). (5) Describe or identify Configuration, if known. (6) Interface / Software equipment integration requirements, if known. (7) Desired condition (new, refurbished or as is / where is). (8) Desired delivery date. (a) If an expedited delivery is required, explain if willing to pay higher price for expedited delivery. (9) Transportation issues. (a) Does Country have Freight Forwarder?? (provide name, address, POC information) (b) Does FF have an Import License?? (c) Is FF cleared for unclassified and or classified?? (d) Provide preference for Delivery Term code (DTC) on LOA and other requirements such as staging spare parts; identify any special transportation requirements. (10) Non-Standard equipment requirements (i.e: require I-level repair facility to be constructed, need CONUS training base, existing METCAL facility needs 19 July 2018 updating, etc). (11) Support

requirements during production. (12) Software development / integration requirements. (13) Locations by name, quantities of where systems/items will be used/stored. (a). Main operating bases (b). Operating bases (c). Repair facilities (d). Weapons maintenance facilities (e). Weapons storage facilities (needed for Expanded End Use Monitoring) (14) Number of detachments; include number of aircraft, duration of detachments, simultaneous detachments / or not. (15) Annual flight hours: Example (300 flying hours per aircraft per year). (16) Operational availability rate (if applicable). (17) Component failure rate (if applicable). (18) Component repair Turn-Around-Time (if applicable). b. If service, please identify as follows: (1) Short description of service requested; Include back-up details, if applicable. (2) Desired length of service; Period of Performance. (3) Purchaser participation; Include number and type of Purchaser personnel available if service’s are to take place in CONUS.

(5) If similar services have been purchased prior to this request under FMS or DCS procurement, identify the FMS case or export license number. (6) Include Sole Source service request, if desired. 2. Additional requirements: a. In terms of logistics support, the following should be addressed: (1) Supply Support. (2) Define spare parts provisioning requirements. The following may apply: (a) Spare parts list. Support periods being covered, eg, 12 or 24 months For end items being supported, provide particulars as to operating plans and program data (Operational Tempo, overhaul periods, maintenance factors, nonstandard repair parts spares/tools, etc.) (b) Training spares. (c) Installation and Checkout spares. (d) Obsolesce / Investment spares. (e) Maintenance float (spares to cover repair turnaround time). (f) Consolidated shipment requirement. (g) Repair and Return (organic or commercial). (3) Planned Maintenance Concept: what level: Organization, Intermediate, Depot. (4) Type of Supply

system to be used by Customer and integration of new US system. (5) Obsolescence plan. b. Engineering Support Requirements: (1) Is there requirements for following services, and if so what duration? (a). Contractor Engineering Technical Services (In-country technical support, i.e: airframes, power plants, avionics/electrical, weapons repair) 19 July 2018 (b). Engineering Technical Services (2) Will USG system / equipment require integration onto a foreign made platform; provide type, model, manufacturer. c. Technical Assistance requirements: (1) Installation, modification, upgrade of systems, equipment in-Country or brought back to CONUS? (2) Post delivery test requirements. (3) Initial/Long Term follow-on maintenance requirements for support. (Organizational, Intermediate, Depot Level). (4) For overhauls and/or upgrades, describe the condition of Purchaser owned equipment. (5) Contractor Logistics Support requirements. (6) Contractor Post Production Support. (7) Contractor Field

Service Support Representative requirements. (8) Repair and Return requirements. (9) Hardware / software testing requirements / software integration laboratory both Contractor / USG requirements. (10) Contractor material augmentation team to augment in-country maintenance personnel. d. Training and Training Devices: (1) Identify Type of training; On-the-Job, Train the Trainer, initial technical training, new equipment training or post-upgraded equipment retraining. (2) Do students meet security, medical, English Comprehension Level requirements. (3) Identify in-Country location(s) where training will be and estimated dates. (4) Identify CONUS locations where training will be and estimated dates. (5) Identify training devices required, if known. (6) Identify any requirement for site surveys. (7) Identify type of training; Pilot, Aircrew, Maintenance, Weapons assembly / loading. e. Documentation Requirements: (1) List any and all documentation (Class / Unclass) to be provided and

quantities. (a) Training, Operating, Maintenance, Repair, Depot level manuals. (b) Support and Test equipment manuals. (c) Safety, System Engineering manuals. (d) Auxiliary equipment / support manuals. f. Support and Test Equipment: (1) General purpose tools and test equipment. (2) Peculiar ground support equipment. (3) Special tools. (4) Calibration services / test equipment and diagnostic equipment. (5) Ancillary equipment. (6) Test measurement and diagnostic equipment. (7) Identify existing test equipment to be used or in need of upgrade. g. Facilities: 19 July 2018 (1) Training Facilities. (2) Storage of equipment (specifically, sensitive category items). (3) New / upgraded facility / construction requirements for aircraft / large equipment. (4) Repair facilities. (5) Identify type of in-country power (240V 2phase, etc). (6) Facility Site Surveys. h. Identify current weapon systems and any interoperability requirements i. Identify any communication or command and control

requirements j. Miscellaneous requirements: (1) Petroleum, oil and lubricants. (2) Cold weather requirements. (3) Basic issue items, (clothing, safety gear items, etc.) (4) Communications/electronics equipment (radios, accessory kits, etc.) (5) Power generators. (6) Specialized software. (7) Special waivers to be requested. (8) Forward Area Refueling/Arming Points. 19 July 2018
